    Hepatic encephalopathy is usually as a result of the accumulation of nitrogenous waste especially ammonia in the central nervous system. This happens commonly because of the inability of the liver to detoxify ammonia and other chemicals.

    The use of loop diuretics is known to cause a decrease in potassium levels and also lead to alkalosis which facilitates the conversion of ammonium (NH4+) to ammonia (NH3). The excessive production of ammonia then causes accumulation in the CNS leading to the encephalopathy.

    Also, diuretic-induced hypovolemia is the most common reason for patients with previously well-controlled hepatic encephalopathy to present to the emergency room with worsening mental function.
